These Terms of Service ("Terms") govern your use of the Test Anywhere website and your engagement of services from Test Anywhere LLC ("Test Anywhere," "we," "us," or "our"). By scheduling a service or using this website, you agree to these Terms.
Test Anywhere provides on-site and in-lab drug testing, wellness blood screening, youth CPR/first aid training, and related services, delivered directly to individuals, employers, and organizations across the Memphis metro and Mid-South.
Requests submitted through this website are appointment requests, not confirmed bookings, until we contact you to confirm date, time, and location. We'll do our best to accommodate your preferred timing but cannot guarantee availability.
Wellness screenings provided through Test Anywhere are for informational and preventive purposes only and are not a medical diagnosis. Always consult a licensed healthcare provider regarding any results or health concerns.
Pricing for services is as listed on our website at the time of booking, or as separately agreed with employer or organizational clients. Payment information you provide is used solely to process charges for the services you request. Payment terms will be confirmed at the time of scheduling.
You may reschedule your appointment free of charge if you notify us at least 24 hours before your scheduled appointment time. Cancellations or reschedule requests made less than 24 hours before the appointment are subject to a $25 fee.
Our youth CPR/first aid training is intended for participants under 18. A parent or legal guardian must submit any request or registration on a minor's behalf, and by doing so represents that they have authority to consent to the minor's participation. This website is not intended for use by minors to submit requests directly.
CPR and first aid training involves physical activity, including practice on manikins and hands-on exercises. Participants (or their parent/guardian, for minors) voluntarily assume the risks associated with this physical activity. Test Anywhere is not liable for injuries arising from normal participation in training exercises conducted in accordance with standard instructional practice.
Where a test is conducted under U.S. Department of Transportation (DOT) regulations, that test is governed by applicable federal DOT rules and procedures in addition to these Terms. Non-DOT tests follow the applicable employer's or requesting party's policy together with our standard collection procedures. It is the customer's or employer's responsibility to specify whether a test is DOT-regulated at the time of scheduling.
Test Anywhere serves as a Labcorp collection site. Specimen collection is performed by Test Anywhere; laboratory analysis and reporting of certain tests is performed by Labcorp under its own procedures and quality standards, which are outside Test Anywhere's direct control.
Health information collected in connection with our services is handled in accordance with our Privacy Policy and applicable HIPAA requirements.
You agree to indemnify and hold Test Anywhere LLC harmless from claims, damages, or expenses arising from your misuse of test or screening results, provision of false or misleading information, or violation of these Terms.
To the fullest extent permitted by law, Test Anywhere LLC is not liable for indirect, incidental, or consequential damages arising from use of our website or services. Nothing in these Terms limits liability that cannot be limited under applicable law.
All content on this website, including text, graphics, and logos, is the property of Test Anywhere LLC or its licensors and may not be copied or used without permission. You agree to use this website only for its intended purpose of learning about and requesting our services.
We reserve the right to refuse, cancel, or discontinue service to any individual or organization, including for safety concerns, non-payment, or misuse of our services.
Any dispute arising from these Terms or our services will be resolved in the state or federal courts located in Shelby County, Tennessee, and you consent to the exclusive jurisdiction and venue of those courts.
These Terms are governed by the laws of the State of Tennessee, without regard to conflict-of-law principles.
If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect. These Terms, together with our Privacy Policy, constitute the entire agreement between you and Test Anywhere LLC regarding your use of our website and services.
We may update these Terms from time to time. Continued use of our website or services after changes take effect constitutes acceptance of the updated Terms.
