Test Anywhere LLC ("Test Anywhere," "we," "us," or "our") provides mobile drug testing, wellness screening, and youth CPR/first aid training services. This Privacy Policy explains what information we collect, how we use it, and the choices you have.

Protected health information & HIPAA

Test Anywhere treats drug test results, wellness screening results, and other health information you provide as Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). We maintain administrative, physical, and technical safeguards designed to protect PHI, limit access to personnel who need it to perform their job, and disclose PHI only as permitted or required by HIPAA — including to you, to an employer or organization that authorized and requires the result, to our laboratory and screening partners who perform testing on our behalf, or as required by law.

Information we collect

When you request a service, contact us, or schedule an appointment, we collect:

Contact form information you submit (name, phone number, email address, and any message you include)
Scheduling information (service requested, preferred date, location)
Payment information needed to process charges for services rendered
Health information (PHI) necessary to deliver the service you request, such as drug test results, wellness screening results, or information relevant to a CPR/first aid class

How we use your information

To schedule, confirm, and deliver the services you request
To process payment for services
To communicate with you about appointments, results, or account matters
To meet legal, regulatory, and recordkeeping obligations tied to testing and screening services, including HIPAA

We do not sell your personal information.

Sharing your information

We share information only: with service providers who help us operate (e.g., laboratory partners and payment processors), when an employer or organization has authorized a test or screening on your behalf, or when required by law or legal process.
